AT A GLANCE
ACHIEVING AHA CERTIFICATION
Bethel Home is the first skilled nursing facility in Wisconsin, and one of 14 in the country, to receive its certification as a Post-Acute Care Heart Failure Center through the American Heart Association (AHA). This program improves the outcomes of patients with chronic heart failure by providing a new standard of care, developed by the AHA and maintained with the most up-to-date research and protocols in the industry.
AFTER THE HEART FAILURE DIAGNOSIS
Although it can be difficult living with a chronic condition like heart failure, with proper management of symptoms, many people enjoy full lives. People who build new habits around eating better, tracking and managing their symptoms, and exercising (as directed by our team) produces positive outcomes. This program ensures we offer the best opportunity possible to return patients to a lifestyle they were familiar with prior to the diagnosis.
